At Churchill Estates our Assisted Living Facility is a very important part of our "Continuity of Care" program. Our goal is to help empower seniors who wish to maintain their independent lifestyle but need some additional assistance.
Churchill Estates Memory Care program is dedicated to the specialized requirements of seniors with memory loss. Our main focus is maintaining the safety, dignity and maximizing personal freedom for our residents who suffer from Alzheimer's and other forms of dementia. We address challenges that are caused as a result of memory loss by providing a comfortable home-like environment. Our residents receive dedicated quality care and services provided by our specially trained caregivers and licensed nurses.
We have full-time activities staff who provide activities that are designed to enhance and retain sensory skills. Our activities staff creates an active day-to-day lifestyle that focuses on what residents are capable of doing rather than activities that have become more challenging. Our activity plans provide the assistance needed to remain safe and active in everyday life. Personal interactions with family are strongly encouraged and monthly support groups are offered.

Fees for personalized care are determined before admission, after our professional licensed staff have met with you and reviewed medical records. These fees are in addition to the basic monthly rate. The nursing staff works closely with each resident and their family members to provide the best care possible. See precise care level pricing below.

Service Level 1: $435
Medication assistance. Resident is independent with all other activities of daily living.
Service Level 2: $656
Includes all the above, plus Stand by Assistance for activities of daily living (mobility, transferring, bathing, toileting, grooming, dressing.) Set-up, cue, reminders and direction for activities, meals, and other events at facility.
Service Level 3: $1,207
Includes all the above, plus occasional hands-on assistance for activities of daily living (mobility, transferring, bathing, toileting, grooming, dressing.) Vision, hearing and communication assistance. Denture care, hearing aid assistance, compression socks, specialized diet, snacks/meals. Blood Glucose monitoring, Nurse delegation treatments. Mild cognitive impairment requiring additional support. Fall risk prevention.
Service Level 4: $1,759
Includes all the above, plus moderate assistance for activities of daily living (mobility, transferring, bathing, toileting, grooming, dressing.) Consistent incontinence care, feeding assistance. Special preparation of medications. Middle stage dementia/cognitive impairment requiring frequent staff assistance and monitoring. Other treatments or therapies.
Service Level 5: $2,310
Includes all the above, plus full assistance with ALL tasks, 24/7 incontinence care. Late stages of dementia requiring constant staff supervision or assistance. Psychological/behavior issues that intefere with care. Additional housekeeping/laundry assistance due to incontinence. Other extensive assistance requiring frequent caregiver intervention, oversight, or time; including those residents receiving Hospice Care.
Please Note:
We do not currently accept insulin-dependent diabetic residents or those who require two-person transfer assist, mechanical lift assist, or those who are non-ambulatory. Care service levels for our assisted care facility are determined by quarterly evaluations by skilled nurses and administrators.
